Arts Award Application

Thank you for your interest in the Chamber’s award program.  Livingston Parish Chamber of Commerce Award Applications are only available to graduating seniors who attend schools in Livingston Parish. 

Please fill out the following application materials and include all necessary materials in your application packet.  We recommend that you return your packet to your counselor for their review of materials and to ensure your application is complete.  An official transcript signed by your counselor is necessary to complete the application.

We also offer you the opportunity to mail or deliver your packet to the Chamber Office.  Zeros will be given for incomplete information/items requested.  Your completed packet should be dropped off or postmarked no later than March 30, 2012. 

Recipients will be awarded for their freshman fall semester.  Scholarships awarded will be in the amount of $1000 each.  All students must retrieve their Arts Award submissions by April 30, 2012.  The Chamber will not be responsible for projects left beyond that date.

April 4^th – Awarded students notified

April 6^th- RSVP due for scholarship presentation luncheon

April 11^th – Chamber Scholarship / Awards Presentation Luncheon with Sponsors

REQUIREMENTS

Your complete packet must be turned in to your school’s Guidance Counselor in order for it to be submitted to the Chamber of Commerce by March 30, 2012.

1. You must be a resident of the State of Louisiana and a graduating senior in a Livingston Parish high school (public, private, parochial, charter or state approved homeschool).

1. You must have a 2.0 (C) overall point average grade from your present school.

1. Completely fill out the Student Information Form. Ask a parent or guardian to sign it.

1. Ask one teacher or professional instructor who knows your talent to fill out the Instructor/Teacher Recommendation Form, and return the completed form to your school Guidance Counselor.

1. Ask your Guidance Counselor to send in your transcript with your application packet.

1. You must provide one performance piece as specified below.

CREATIVE WRITING

Include with your completed application: Sample composition of up to 500 words (typed) of your own poetry or fiction that you consider to be your best work.

DANCE

Include with your completed application:  A video of up to 5 minutes of your performing a dance routine that is not your choreography.

MUSIC: Instrumental or Vocal

Include with your completed application: A video or audiotape of up to 5 minutes of your performing a work of a major composer.

THEATRE

Include with your completed application:  A video of your performing (not reading) a soliloquy from a major play. 

VISUAL ARTS

Include with your completed application:  An original piece of art.
